Initial Consultation Paperwork
If you have an initial consultation scheduled with us via phone or Skype, then we’ll need to have you complete some paperwork first. There are links to the 3 necessary documents below. Each of the links will take you to a secure form service on the FormStack site.
Please note that your credit card will not be charged until after your initial consultation.
It’s important that we have these documents completed at least an hour before your scheduled initial consultation. If you have trouble with any of it, give us a call at 512-481-0330.
